About us

About the role

The NDIS Transition Team is made up of 11 NDIS Transition Leaders which are based regionally across NSW and the ACT. The team assist current and future Participants in a smooth transition to the NDIS, then again each year through the transition into consecutive NDIS plans.

The following are some of the key responsibilities in this role:

  • NDIS enquiries internal and external
  • Preplanning
  • Attending NDIS and/or LAC planning meetings
  • NDIS document preparation meetings with Participants to discuss and agree on supports to be provided by The Disability Trust
  • Plan Reviews
